Submit Annual Club Information Form

At the beginning of the Circle K year, it is the Club Secretary’s job to submit the Club information to the District. We have made this easier for you by posting a form on the District website so that you can submit the required information to the District. This form includes the phone number, fall and summer address, and email address for all of your Club Officers, and Advisors. Other information that is required for this form includes Club meeting information, and questions regarding your Club Bylaws. If you have not already submitted this form for your Club, then please do so as soon as you can. This will allow for the District to easily contact your Club when needed.
